What First Responders Face

The Need: First Responders Are in Crisis

In the United States, an estimated 4 million first responders serve in roles such as firefighters, law enforcement officers, EMS personnel, paramedics, and emergency dispatchers. These professionals are repeatedly exposed to violence, tragedy, death, and human suffering as a routine part of their work (SAMHSA, 2018).

Approximately 30% of first responders develop behavioral health conditions such as depression or PTSD compared with about 20% of the general population (SAMHSA, 2018). Studies show 7–37% of first responders report symptoms consistent with PTSD (Berger et al., 2012). In some years suicide claims more firefighter and law enforcement lives than line‑of‑duty deaths (Ruderman Family Foundation, 2018; Firefighter Behavioral Health Alliance, 2022). Occupational trauma is strongly associated with depression, anxiety, sleep disorders, substance misuse, and burnout (Violanti et al., 2017; Stanley et al., 2016).

Barriers to Care

First responders frequently report stigma, concerns about career impact, and difficulty finding culturally competent mental health providers as major barriers to treatment. As a result, many delay seeking help or discontinue treatment early (Haugen et al., 2017).

Our Solution: A 40‑Day Residential Healing Program

Still Waters Ranch is a 40‑day residential recovery program designed specifically for first responders. The ranch environment removes participants from operational stress while offering quiet, structure, nature, and community for restoration.

Core components include evidence‑based trauma therapy, faith‑informed spiritual care, equine‑assisted therapy, physical fitness and nutrition support, peer connection, family reintegration programming, and structured aftercare planning.

Evidence Base

Clinical guidelines from the U.S. Department of Veterans Affairs and Department of Defense identify trauma‑focused psychotherapies such as Cognitive Processing Therapy (CPT), Prolonged Exposure Therapy, and Eye Movement Desensitization and Reprocessing (EMDR) as first‑line treatments for PTSD (VA/DoD Clinical Practice Guideline, 2023).

Equine‑assisted therapy has also shown promise as a complementary trauma intervention. Research suggests interactions with horses can improve emotional regulation, reduce anxiety, and strengthen interpersonal trust among trauma survivors (Pendry & Roeter, 2013). Studies involving veterans with PTSD demonstrate improvements in psychological well‑being, social functioning, and stress regulation when equine‑assisted programs are integrated alongside traditional therapy (Johnson et al., 2018).

Who We Serve

Still Waters Ranch serves law enforcement officers, firefighters, EMS professionals and paramedics, 911 dispatchers, corrections officers, Emergency Physicians, Registered Nurses, retired or medically separated first responders, and spouses and family members through designated programming. Participants are welcome regardless of faith background, and financial assistance is available when funding allows.

Theory of Change

If first responders receive intensive trauma therapy, peer community, spiritual care, time away from operational stress, and structured aftercare, they will experience reduced PTSD symptoms, improved functioning and relationships, decreased suicide risk, and renewed purpose.
